OFAI Forms

All forms that are required prior to your assessment(s) must now be accessed and completed online in your NEW Frontline Health and Performance Smartabase account at https://frontlinehp.smartabase.com/health.

To access the Frontline Health and Performance Smartabase account for the first time, you must follow these steps:

  1. Go to: frontlinehp.smartabase.com/health
  2. In the “Username or email” field, enter either your six-digit OFAI ID number or your email address used to register with OFAI.
  3. On the ENTER PASSWORD screen, select “Forgot password”.
  4. Enter the email address you used to register with OFAI in the field provided.
  5. You will receive an email requiring you to set a new password.
  6. Login to your Frontline Health and Performance Smartabase account using your Username (email or six-digit OFAI ID) and new password. You have now successfully set-up your Frontline Health and Performance Smartabase account.
  7. Visit the START HERE page under the “Other/My Resources” section for information on how to navigate the Frontline Health and Performance Smartabase platform.

If you are having trouble logging into your Frontline Health and Performance Smartabase account, you can still attend your OFAI assessment(s) The evaluation staff onsite will assist you with the login process and the completion of forms.

OFAI Candidate Agreement and Assumption Waiver, Release, and Indemnity.

This form must be completed by all candidates prior to the commencement of any assessment. Please ensure you read the form in full and complete ALL applicable areas of the form.

Medical Questionnaire

This form must be completed by all candidates taking the Encapsulated Treadmill and Stage Three assessments. This form is required prior to the commencement of the assessments. This form is NOT required for Stage One and Stage Two Hearing Assessments.

Medical Clearance Form

If a candidate answers YES to any of the eight (8) questions on the Medial Questionnaire, this clearance form must be completed by a physician before engaging in candidate testing.

Physician Information Booklet

This booklet must be presented to the physician along with the Medical Clearance Form before engaging in candidate testing.

Informed Consent for Clinical Evaluation

This form must be completed by all candidates taking the Encapsulated Treadmill Test. This form is required prior to the commencement of the Encapsulated Treadmill Test.

Informed Consent for Firefighter Physical Aptitude Job-Related Tests

This form must be completed by all candidates taking the Firefighter Physical Aptitude Job-Related Tests (FPAT). This form is required prior to the commencement of the FPAT.

Vision Assessment Form

Candidates must meet the vision requirements as outlined in Chapter Six of NFPA 1582: Standard on Comprehensive Occupational Medical Program for Fire Departments.

The vision assessment is NOT conducted by OFAI at our test facility. Candidates must take the vision assessment form (located here and on the Frontline Health and Performance Smartabase platform) to an eye care professional of their choosing for completion and pay the applicable fees associated. Candidates may also have the option to utilize personal extended health care benefits, if available.

Candidates must submit the completed form in one PDF file to your Smartabase account. Once you are logged in, click the My Resources button located on the home page. Click on the resource titled Start Here and follow the instructions accordingly.

If you do not follow the instructions on the Start Here page in full, your vision form will not be processed.

OFAI will ONLY accept our vision form, and it must be stamped by your optometrist. Please DO NOT attach a business card – if a stamp is unavailable, please attach your paid receipt. Failure to submit correct documentation will result in your vision form not being processed. You may be required to return to your optometrist and additional charges may apply.

Please allow a minimum of three (3) business days for your vision form to be processed.
